Volunteer-Staff Application Form – If you are an interested volunteer, fill out this application form and bring it with you when visiting the riding center.
Rider’s Application and Release Form – Parents/guardians of future participants, please fill out and bring with you to first time meeting.
Participant’s Medical History and Physician’s Statement – Parents/guardians of participating students must have this signed by a licensed physician before attending classes.
Authorization for Emergency Medical Treatment Form – For students and/or volunteers. Must fill out prior to attending classes.
